Special Notice: State Health Benefit Plan Beneficiaries that have elected Medicare Advantage coverage through United Healthcare—

Effingham Hospital, Inc. d/b/a Effingham Health System is available to provide healthcare services for State Health Benefit Plan Beneficiaries that have elected Medicare Advantage coverage through United Healthcare. It is Effingham’s understanding that the United Healthcare benefits for State Health Benefit Plan Beneficiaries that maintain Medicare Advantage plan coverage are the same whether the provider is “in-network” or “out of network”. Therefore, based upon this understanding, although Effingham is out of network with United Healthcare, the benefit coverage for the State Health Benefit Plan Beneficiaries that participate in the United Medicare Advantage plan is the same as in-network provider benefits.
